Summary of Employee Benefits - Plan Year 2012

All U.S. locations except Puerto Rico

The following health and welfare benefits are offered at PTGi. Eligibility to receive these benefits vary by employee type, number of hours scheduled to work, and work location. Each of the eligibility requirements are outlined in the respective Summary Plan Descriptions. These plans are identified below, along with the associated semi-monthly cost via pre-taxed payroll deductions.

PTGi offers both Healthcare and Dependent Care Flexible Spending Accounts.

Healthcare
- $600 Minimum/$5,000 Maximum
Dependent Care - $360 Minimum/$5,000 Maximum, if filing jointly, filing separately, $2,500 maximum

Ancillary Benefits – Reliance Standard Life Insurance Company

PTGi provides the following coverage at no cost to you:
  • Accidental Death & Dismemberment Insurance – Two times your annual base salary up to a maximum of $330,000.
  • Employee Life Insurance – Two times your annual base salary up to a maximum of $330,000.
  • Spousal and Dependent Life Insurance – Up to $5,000 for spouse and $2,000 per eligible dependent.
  • Short Term Disability - 66 2/3% of your weekly salary up to a maximum of $1,500.
  • Long Term Disability – 66 2/3% of your monthly salary up to a maximum of $20,000.

401(k) PlanPrincipal Financial

PTGi offers a 401(k) Plan that allows you to save for the future and helps you supplement your retirement income. PTGi matches 50% of the first 6% of salary deferred, with an annual maximum match of $6,000. PTGi matches on a semi-monthly basis as allocated by your current investment elections.  The plan offers a Roth 401(k) option and permits catch-up contributions for participants age 50 and above.

Paid-Time Off

The amount of PTO any regular employee can receive in one calendar year increases with the length of employment as shown with the following schedule:
  1. Beginning on the employee’s original date of hire through year three (3) of service – 17 days annually, accrued at a rate of 5.66 hours per pay period.
  2. After three (3) years of service from the employee’s anniversary date – 22 days annually, accrued at a rate of 7.33 hours per pay period.
  3. After nine (9) years of service from an employee’s anniversary date – 27 days annually, accrued at a rate of 9.00 hours per pay period.
Regular, part-time employees accrue PTO based on the numbers of hours they are regularly scheduled to work.
